Calendar pattern time zone symbol fallback behaviour

It is not sufficient in the spec just to list the symbols and describe them in the current simple way, as there is often a fallback behaviour if a particular symbol does not have a meaning in a particular time zone.

Changes required to section 13.11.1:

1) Sentence "The pattern is derived from the ICU SimpleDatetimeFormat class described here: [ICUCalForm]" should append "which uses symbols defined by [UnicodeLDML]"

2) A note should be added at the foot of the calendar pattern symbol table which says:
"If a time zone symbol is not available for a particular time zone, a fallback may be used, as defined in [ICUCalForm]."

Change as described, but clarify point 2 with "When unparsing..."

"When unparsing, if a time zone symbol is not available for a particular time zone, a fallback may be used, as defined in [ICUCalForm]."

The errata doc v1.1 says to add a footnote to the table. I considered this and decided a footnote number hanging on a table was not prominent enough, so I simply added the suggested sentence to the prose in an appropriate spot. (Change bars will show where.)
