Appendix B - should this be in the spec at all?

Seems like Appendix B belongs in some advanced tutorial.

Material will be moved out to a tutorial.
A listing of all the places where dfdl:encoding applies will be added to description of the dfdl:encoding property.
(also the utf16Width property, which modifies encoding)
